How to become a member?
We would like to inform you that, notwithstanding 10 years of invariable fees, the Board of the Valsè Pantellini Foundation has approved an increase in membership contributions in order to sustain the burden of expenditure that has become more substantial both in terms of (extra)ordinary administrative and management costs as well as the development of its research projects. The increased membership fees are effective from 1 January 2014.
Not having other economic resources than those related to the payment of membership fees, the Board has decided to augment the standard annual contribution from 150 to 200 euros and the reduced annual contribution from 75 to 100 euros.
We apologize in advance for the inevitable inconvenience that might be created in these generally difficult economic times. The staff of the Foundation will remain, as always, at its members and anyone else’s full disposal for further clarification.
The Pantellini Foundation has determined that the membership contribution for a standard member is 200 euros and valid for 12 consecutive months from the date of payment, without any obligation for renewal. This kind of membership is necessary in order to comply with the provisions of the law on the treatment of confidential information (authorization for the release of information under the privacy act) and is related to the medical consultation service at the Foundation.
The Pantellini Foundation has also agreed that for people who wish to associate themselves for the reason of prevention, i.e. without the presence of a proclaimed or previous disease, there is the possibility to enjoy a reduced membership fee of 50%, which will then become 100 euros.
The Foundation offers free access to the service of accredited medical consultation, in particular with doctors who offer their services at the Foundation’s office in Florence, to all its associates throughout the period of valid membership.

In order to join the Pantellini Foundation, please complete both above-mentioned forms, sign them and send us the original documents for our administration (a copy of the documents must be kept by the applicant).
The second form has to be completed and signed in order to comply with the provisions of the law regarding the treatment of confidential information (authorization for the release of information under the privacy act). The ‘applicant association’ part must be completed in case the contact person for our doctors is not the same person as the associate (i.e. the directly interested person).
Important: please note that in order to finalize the request for membership, BOTH the Membership Application form AND the Activity Consent and Approval Form have to be duly completed, signed and sent to us; we can no longer take into account incomplete membership requests.
